Initially only the form is to be sent to Stericycle - no drugs are to be shipped until advised.
Stericycle will review the data on the analysis form and will forward a DEA Form 222 authorization to ship the expired Class II drugs for processing.

Step by Step Instructions

• Step 1: Start with your Stericycle account number and your pharmacy's 9-digit DEA number;
• Step 2: Enter Class II pharmaceuticals one by one, starting each new item with the NDC #;
• Step 3: Receive a confirmation number, print a copy of the DEA 222 Request.
